Bone deficiency in revision total hip arthroplasty (THA) is a challenge to the surgeon. The management of severe acetabular deficiency at revision arthroplasty, complex primary total hip replacement after postinfective or post-traumatic sequelae remains a complex problem. The purpose of the acetabular reconstruction is to obtain a stable, durable fixation of a new socket to reestablish the center of rotation and, if possible, restore bone stock.
There are different options to overcome the problem of acetabular deficiency. Porous-coated cementless acetabular cups or cemented cup fixation can be used with or without a cancellous bone graft for simple acetabular defects, either cavitary or segmental.
